My parents/guardians are aware I am submitting this application. I understand that if my application is successful, acceptance and indemnity forms will be sent to me and one of my parents/guardians will be required to provide their details and signature.

I acknowledge that I will have to organise my own transport to and from the pick up / drop off point.I understand that if my application is successful, I will be required to pay a $200 deposit to secure my place at camp, $100 of which will be refunded at the end of the camp.
